Contact our Admissions Department at (850) 656-4863 or (850) 656-4867. One of our Rehabilitation Liaisons will respond promptly to perform a pre-admission screening, coordinate transfer to our facility or provide a tour of the hospital or more information.

Home Evaluation
The hospital provides no cost home screenings for individuals who might benefit from rehabilitation. To facilitate a screening, the individual, a family member or a healthcare professional should call (850) 656-4863 to request a home evaluation. A liaison will visit the patient's home, in order to complete the screening and make appropriate recommendations.

Insurance
Rehabilitation is covered in full or part by most health insurance plans, including Medicare and Medicaid. The hospital has contracts with most major insurance companies and is willing to negotiate with other companies as needed.
