What is the Case Manager’s Role in Inpatient Rehabilitation?

Your hospital case manager plays an integral role in the rehabilitation and recovery process. The case manager meets daily with patients and family to coordinate all aspects of care and arrange family conferences based on the recommendations of the treatment team or by family request. Family conferences may be held with the hospital’s interdisciplinary team members to discuss therapy goals, discharge plans and other patient centered issues.

The case manager’s role does not end at discharge. Case managers may continue to call a patient after they have left the hospital to see how the patient is doing. We want to make sure that our patients are comfortable using whatever new therapies or technologies are suggested for them and return to an active lifestyle.

Becoming a Patient

Our hospital provides care for those who are ready to be discharged from a hospital but not ready to return home. Treatment and care is designed to bridge the gap between hospital and home. Anyone who is limited functionally from an injury or illness can benefit from rehabilitation.
