Social Workers Help Family in Return to Home

Our hospital has social workers that help patients at different stages of the rehabilitation process. Many times when a patient is ready to return home they may have to do so with slight modifications to their home or with adaptive equipment. We are there to help with this process and to make sure that the patient experiences a seamless continuum of care.

Becoming a Patient

Our hospital provides care for those who are ready to be discharged from a hospital but not ready to return home. Treatment and care is designed to bridge the gap between hospital and home. Anyone who is limited functionally from an injury or illness can benefit from rehabilitation.
